Make an appointment

We see patients by appointment only. Same-day appointments are often available. To schedule an appointment, call our main line at 706-922-0440.

We may experience some scheduling delays. We will make every attempt to notify you before your appointment if we are running behind schedule.

Cancel or reschedule appointment

We ask you call our offices at least 24 hours before your scheduled appointment to cancel or reschedule.

Emergencies and after-hours appointments

If you are experiencing a medical emergency, please call 911.

If you need advice about your care after office hours, please contact us, and our answering service will assist you.

New patients

If you're new to our practice, please arrive 15 minutes before your first appointment to complete paperwork. You can save time by downloading, printing and completing patient forms prior to your appointment.

Please bring the following items to your appointment:

  • Completed forms
  • Insurance card and information
  • List of current medications
  • Recent copies of X-rays and any lab results

Insurance

Before your first appointment with us, we recommend you call your insurance company to confirm that your insurance will cover our services. We submit service claims to most major insurance carriers.

Please bring your insurance card and identification with you to every visit, and let us know if you have had a change in your name, phone number, address or insurance coverage.

Billing and payments

Any insurance co-payments or past-due account balances must be paid at the time of your visit. We accept debit, personal checks and all major credit cards. To view your account records and billing summary, log into our patient portal.

You can also use our secure online bill pay option to make a payment online without setting up a patient portal account.

Prescription refills

Contact us during regular office hours to request any prescription refills.
